Human trial data for NMN is thinner than for NR but growing, with recent Japanese trials showing improvements in muscle function and insulin sensitivity in older adults at 250 to 500 mg daily.<sup>[5]<\/sup> Cost per month at effective doses is typically \u00a510,000 to \u00a525,000, with a wide quality range in the Japanese market \u2014 many low-cost NMN products contain substantially less NMN than the label claims, based on independent third-party testing.<\/p>\n<h3>Lifestyle levers that support NAD+<\/h3>\n<p>Three lifestyle interventions have documented effects on NAD+ metabolism and are, in effect, free:<\/p>\n<ul>\n<li><strong>Regular moderate exercise<\/strong> increases NAD+ salvage pathway activity in muscle. This is one of the most reliable non-pharmacological levers.<sup>[6]<\/sup><\/li>\n<li><strong>Time-restricted eating and intermittent fasting<\/strong> upregulate the enzymes that recycle NAD+, particularly SIRT1 activity.<\/li>\n<li><strong>Adequate sleep<\/strong> \u2014 the NAD+\/NADH balance is directly linked to the circadian rhythm, and chronic sleep restriction depresses NAD+ metabolism.<\/li>\n<\/ul>\n<p>A patient who is sedentary, chronically under-slept and skipping meals irregularly will get more mileage from correcting those foundations than from any injectable therapy.<\/p>\n<h2>Who should consider NAD+ IV and who probably should not<\/h2>\n<h3>Reasonable candidates<\/h3>\n<ul>\n<li>Patients over 40 with a longevity focus who have already optimised sleep, exercise, diet and known nutritional deficiencies and want to add a targeted therapy.<\/li>\n<li>Patients recovering from a prolonged viral illness or post-COVID with persistent fatigue that has not responded to nutritional correction.<\/li>\n<li>Patients who have tried oral NR or NMN with subjective benefit and want to explore whether the intravenous route produces a stronger response.<\/li>\n<li>Patients with the discretionary budget to trial a therapy that may or may not work for them, and the psychological willingness to accept &#8220;no effect&#8221; as a possible outcome.<\/li>\n<\/ul>\n<h3>Poor candidates<\/h3>\n<ul>\n<li>Patients with untreated iron deficiency, vitamin D deficiency, thyroid dysfunction or other correctable nutritional or hormonal problems. Fix those first. NAD+ will not compensate for a ferritin of 15 ng\/mL.<\/li>\n<li>Patients with a specific medical diagnosis (major depression, Parkinson&#8217;s disease, chronic fatigue syndrome) who are hoping NAD+ will treat the condition. It is not an approved treatment and should not be positioned as one.<\/li>\n<li>Patients pregnant or breastfeeding \u2014 the safety data in these populations is insufficient.<\/li>\n<li>Patients on chemotherapy or with active cancer without oncology clearance \u2014 the interaction of NAD+ with tumour metabolism is an active research question and not settled.<\/li>\n<li>Patients with a modest budget who would be better served by a nutritional workup and targeted supplementation at a fraction of the cost.<\/li>\n<\/ul>\n<h2>Preparing for an NAD+ IV appointment<\/h2>\n<p>If you decide to proceed, preparation improves both tolerability and the value you get from the therapy.<\/p>\n<h3>Hydrate the day before<\/h3>\n<p>NAD+ infusions are better tolerated in a well-hydrated patient. Drink at least 2 litres of water the day before your appointment, and continue during the infusion itself.<\/p>\n<h3>Eat a light meal 2 to 3 hours before<\/h3>\n<p>Unlike a nutritional blood draw, an NAD+ IV does not require fasting. A light meal with some protein and complex carbohydrate reduces the nausea and lightheadedness that can occur during rapid infusion.<\/p>\n<h3>Bring your medication and supplement list<\/h3>\n<p>The physician overseeing the infusion needs to know what else you are taking. Several categories interact with NAD+ metabolism or with the infusion process itself \u2014 some psychiatric medications, some cardiovascular medications, and high-dose antioxidant supplements can all affect how the infusion is tolerated.<\/p>\n<h3>Clear the afternoon<\/h3>\n<p>An NAD+ infusion takes 90 minutes to 4 hours. Add 15 to 30 minutes on either side for check-in and post-infusion observation. Do not schedule a meeting or a workout immediately after.<\/p>\n<h3>Set realistic expectations for the infusion itself<\/h3>\n<p>Chest tightness, a heated flushing sensation, mild nausea and abdominal cramping are all normal and expected during rapid delivery. Tell the nurse if any sensation is unbearable \u2014 slowing the drip resolves it within minutes. It exists because in my clinical view NAD+ IV is a legitimate but oversold therapy, and patients deserve to know when their money would produce better results in a different intervention entirely.<\/p>\n<h2>Frequently asked questions<\/h2>\n<h3>How much NAD+ IV can I expect per session?<\/h3>\n<p>Standard sessions in Tokyo deliver 250 mg to 1,000 mg of NAD+ per infusion, with 500 mg being the most common single-session dose. Clinics offering &#8220;starter&#8221; sessions typically deliver 100 mg to 250 mg at a lower price point.<\/p>\n<h3>How long does the infusion actually take?<\/h3>\n<p>Slower infusion is better tolerated. Practical range is 90 minutes for a 250 mg starter dose to 3 or 4 hours for a 1,000 mg dose. Rapid infusions of 500 mg over 30 to 45 minutes are technically possible but produce marked flushing and chest tightness that most patients do not enjoy.<\/p>\n<h3>Is NAD+ IV covered by Japanese health insurance?<\/h3>\n<p>No. NAD+ IV is classified as jihi shinryo (private-fee care) and is not covered by kokumin kenko hoken, shakai hoken or standard international insurance policies that follow Japanese classification. The full fee is paid at the point of care.<\/p>\n<h3>How soon after the infusion will I feel something?<\/h3>\n<p>If you are a responder, you may notice increased mental clarity and physical energy within 24 to 48 hours, with an effect that persists for one to three weeks depending on your baseline. Non-responders notice nothing. There is no reliable way to predict which category you fall into before trying.<\/p>\n<h3>Can I combine NAD+ with glutathione or vitamin C in the same infusion?<\/h3>\n<p>Combination drips are commonly offered. The main downside is attribution \u2014 if the combination produces a benefit, it is difficult to know which component was responsible. If your goal is to evaluate NAD+ on its own merits, a single-agent trial is more informative.<\/p>\n<h3>Is NAD+ IV safe long-term?<\/h3>\n<p>Short-term safety in healthy adults appears reasonable based on the available clinical experience. Long-term safety data at monthly-maintenance dosing over years is not yet well-characterised. This is a common situation with newer therapies and does not by itself preclude use, but patients should be aware that &#8220;long-term safe&#8221; is not yet formally demonstrated.<\/p>\n<h3>What about NMN or NR instead?<\/h3>\n<p>Oral NR has the strongest published human trial data of any NAD+ intervention and is meaningfully cheaper than IV therapy. For a patient wanting to trial NAD+ elevation on a modest budget, oral NR at 300 to 600 mg daily for 3 months is a reasonable starting point. NMN is also worth considering, with the caveat that product quality varies substantially in the Japanese market.<\/p>\n<h3>How does BIOTOPE decide whether to recommend NAD+ IV?<\/h3>\n<p>We look at the nutritional panel first. If the panel shows correctable deficiencies that are likely contributing to the presenting symptom, we recommend addressing those first \u2014 usually at a fraction of the cost. If the panel is essentially clean and the patient has already optimised sleep, exercise and diet, we discuss NAD+ IV as one of several options, alongside oral precursor supplementation and other targeted therapies. We do not push loading courses.<\/p>\n<h3>What if I want to try NAD+ IV but I do not live in Tokyo?<\/h3>\n<p>The infusion must be delivered in person and does not lend itself to travel-and-return arrangements at scale. A patient visiting Tokyo for a week can reasonably fit a single-session trial into that time, but a full loading course requires either extended stay or delivery through a home-country clinic. Oral NR or NMN is the more travel-friendly option.<\/p>\n<h2>How to decide<\/h2>\n<p>Three questions settle it for most patients.<\/p>\n<ol>\n<li><strong>Have I addressed the correctable nutritional and hormonal problems that could be driving my fatigue?<\/strong> If not, do that first. NAD+ IV is a poor substitute for iron repletion in an iron-deficient patient, or vitamin D repletion in a deficient one.<\/li>\n<li><strong>Is my budget flexible enough that a course of NAD+ IV that produces no felt benefit will not be a source of regret?<\/strong> Not every patient responds.
