- Zygomatic reduction (cheekbone reduction): the malar prominence is reduced via combined intraoral and small preauricular incisions, with an L-shaped osteotomy that moves the cheekbone medially.
- Reduction genioplasty (chin reduction): the chin is reshaped via an intraoral incision, either by trimming bone or by an osteotomy that repositions the chin.
These are performed under general anaesthesia, usually as a single combined session for patients seeking a comprehensive V-line result. Because the surgery involves osteotomy (cutting bone), swelling and bruising are more substantial and last longer than any soft-tissue-only procedure.
How Painful Is Facial Contouring Surgery?
During the procedure: nothing — general anaesthesia is standard.
Post-operative pain is moderate and best managed with a combination of intravenous analgesia for the first 24 to 48 hours and oral medication afterwards. Most patients rate the peak pain at 4 to 6 out of 10, easing to 2 to 3 out of 10 by day 4 to 5. Ice packs on the cheeks and jawline in 20-minute cycles for the first 48 hours reduce both pain and swelling significantly.
What patients often describe as harder than pain is:
- Restricted mouth opening (trismus) for the first 2 to 3 weeks — chewing is limited to soft food only
- Numbness of the lower lip, chin, or cheek from stretch on the mental or infraorbital nerve, which can last weeks to months (usually resolves fully, though occasionally partial numbness persists)
- Difficulty smiling naturally for the first 3 to 4 weeks
- Speaking clearly is difficult for the first week due to swelling
The Realistic Recovery Timeline
Day 0 (day of surgery): Wake in the recovery room. Face wrapped in a compression bandage from chin to top of head. Significant swelling begins immediately. Overnight hospital stay standard.
Day 1 to 3: Swelling peaks — the face can look nearly unrecognisable to patients seeing themselves for the first time. Bruising extends down the neck to the chest via gravity. Liquid diet only. Speaking painful and unclear. Ice compresses continuously.
Day 4 to 7: Swelling starts to soften. Bruising darkens then begins to fade at the edges. Soft food (yogurt, porridge, tofu) introduced. Compression garment worn under clothes throughout the day.
Day 7 to 10: Sutures removed at the clinic. Facial shape starts to be recognisable. Bruising fades to yellow. Compression garment continues for another 2 to 3 weeks (day and night for the first 2 weeks, then night-only).
Week 2: After about 15 days, the majority of visible swelling has resolved to observers, though the patient still sees swelling. Comfortable in public with mask; soft food still preferred.
Week 3 to 4: Most visible swelling resolved. Numbness of the lip and chin usually starts to improve. Return to soft-normal diet. Face still slightly firmer than final result.
Month 2 to 3: Final visible contour approaches. Numbness continues to improve week by week.
Month 6: Approximately 95% of final result visible. Any residual numbness usually fully resolved.
Month 12: Full internal healing complete. Bone remodelling stabilised.
Aftercare That Matters Most
First 48 hours (in hospital or immediately after):
- Continuous compression bandage or garment.
- Ice compresses on cheeks and jawline (not directly on wounds inside the mouth).
- Head elevated at 45 degrees during sleep.
- Liquid diet only (protein shakes, clear soups, smoothies with a straw).
- Mouth rinses with clinic-provided antiseptic solution after every liquid intake.
- Prescribed antibiotics and pain medication as directed.
Day 3 to 14:
- Compression garment during the day.
- Gentle mouth rinses 4 to 6 times daily.
- Introduce soft food from day 4 to 5 (yogurt, mashed potato, porridge, well-cooked pasta).
- Sleep with head elevated.
- No exercise beyond gentle walking.
- No smoking, no alcohol.
- Attend the day 5 to 7 suture removal appointment.
Week 2 to 4:
- Compression garment at night only.
- Return to soft-normal diet — avoid hard food (nuts, tough bread crusts, raw carrots) for 6 weeks.
- Light exercise from week 2 to 3; running from week 4; heavy lifting or contact sports from week 8.
- Sauna, onsen from week 4.
Week 4 to 12:
- Return to full normal diet as tolerated.
- Gentle facial exercises to encourage nerve recovery if numbness persists.
- Photograph documentation for follow-up.
When Can You Fly After Facial Contouring Surgery?
Air travel is medically safe from day 7 to 10 for short flights, but comfort and safety strongly argue for a longer wait.
- Short-haul (under 3 hours): from day 10 to 14.
- Medium-haul (3 to 6 hours): from day 14 to 21.
- Long-haul (6+ hours): we recommend day 21 to 28 minimum.
Two considerations drive the longer recommendation:
- Deep-vein thrombosis (DVT) risk. Any major surgery elevates clotting tendency for weeks. Long flights combined with recent osteotomy carry a real DVT risk. Standard mitigations: aisle seat, walking every 90 minutes, calf exercises, hydration, and compression stockings.
- Access to same-day clinic care. Late swelling, wound issues, or asymmetry are much easier to assess at the operating clinic than at a home-country ER unfamiliar with your surgical plan.

- Absolute minimum for the surgical timeline: 14 nights. Covers surgery, overnight hospital stay, suture removal, and initial swelling reduction.
- Recommended for most international patients: 21 nights (3 weeks). Enough for major swelling to resolve, for the surgeon to confirm no early complications, and for a safe long-haul flight.
- Recommended for combined jaw + cheekbone + genioplasty V-line: 21 to 28 nights minimum. This is a major surgical event.
The most common planning mistake is treating facial contouring as if it were rhinoplasty. Patients who book 10-day trips have to fly home at peak visible swelling and with sutures possibly just removed — a genuinely miserable travel experience, and one that limits our ability to intervene if any early issue arises.
What About the “V-Line” Combined Surgery?
When patients ask for “V-line surgery,” they usually mean the combination of mandibular angle reduction plus reduction genioplasty (and sometimes zygomatic reduction), performed in a single anaesthetic session. This is technically demanding, extends operating time to 4 to 6 hours, and produces the most dramatic facial contour change of any cosmetic procedure.
Recovery for combined V-line is meaningfully longer than any single procedure:
- Peak swelling: days 3 to 5 rather than day 2
- Bruising: extends to the neck and upper chest, resolves at week 3 to 4
- Compression garment: 4 to 6 weeks total wear
- Return to work timeline: minimum 3 weeks for desk-based work, 6 weeks for on-camera or physically demanding work
- Recommended minimum stay in Tokyo: 21 to 28 nights
Combined V-line is not something to fit around a work trip. Plan a dedicated stay.
Common Concerns From International Patients
“Will I look normal when I fly home?”
By day 14 to 21, most patients look presentable in public with a mask. By day 21 to 28, without the mask you look slightly puffier than usual but not obviously post-surgical to casual observers.
“Will my numbness resolve?”
In most cases, yes — usually within 3 to 6 months. Persistent numbness beyond 12 months is uncommon but possible, most often affecting a small patch of the lower lip or chin.
“Will my smile look different?”
Temporarily, yes — for the first 3 to 6 weeks the smile can look asymmetric or effortful due to swelling and restricted muscle function. Baseline smile typically returns by month 3.
“Can I combine facial contouring with double eyelid surgery in one trip?”
Yes, but the trip length is dictated by the facial contouring, not the eyelids. Combining does not shorten the overall stay.
Warning Signs
Contact the clinic urgently if you notice:
- Sudden increase in swelling or bleeding beyond the expected timeline
- Fever above 38.5°C
- Bad taste or discharge from the intraoral incision (possible infection)
