If you take finasteride for male pattern hair loss, moving to or visiting Japan raises a practical question: will customs let your tablets through, and how many can you pack?

Japan’s rules are specific. Getting them wrong can mean delays at the airport or a parcel held at the post office.

This article explains how finasteride is regulated in Japan, how much you can bring without paperwork, when you need an Import Confirmation certificate, and how to continue treatment with a prescription in Tokyo.

Finasteride is an oral medicine that blocks type II 5-alpha-reductase. This enzyme converts testosterone into dihydrotestosterone (DHT), the hormone that gradually shrinks hair follicles on the crown and front of the scalp in men with androgenetic alopecia, the medical term for male pattern hair loss.

Approval and classification in Japan

Finasteride was approved in Japan in 2005 under the brand name Propecia, and generic versions are also sold. Its approved use is slowing the progression of male pattern hair loss in adult men. The current Japanese package insert (revised August 2023) classifies it as a prescription drug and also as a gekiyaku, a legal category for potent drugs.

The standard Japanese dose is 0.2 mg once daily. It can be increased if needed, up to a maximum of 1 mg per day. In the United States, the labelled dose is 1 mg once daily.

Finasteride for hair loss is not listed on Japan’s National Health Insurance drug price list, so it is prescribed on a self-pay basis. It is not a narcotic or stimulant. It also does not appear on the Ministry of Health, Labour and Welfare (MHLW) lists of products that need ministry confirmation regardless of quantity. The standard personal-import rules for prescription drugs, described below, therefore apply.

What the evidence shows

The Japanese Dermatological Association’s 2017 hair-loss guideline gives oral finasteride its highest recommendation grade (A) for male pattern hair loss and grade D (should not be used) for female pattern hair loss. The guideline summarises a 48-week randomised trial in 414 Japanese men. On scalp photographs, 58% of men on 1 mg and 54% on 0.2 mg showed at least mild improvement.

The pivotal studies in the US prescribing information show a similar pattern. At 12 months, 14% of men taking finasteride 1 mg had any decrease in hair count from baseline, compared with 58% of men on placebo. The same label notes that stopping treatment reverses the effect within 12 months. That is why an unplanned gap in supply during a move matters.

Under Japan’s Pharmaceuticals and Medical Devices Act, importing medicines is restricted. Personal imports for your own use are allowed within set quantities, and MHLW states that this includes medicines you bring back from abroad in your luggage.

The one-month rule

For prescription drugs such as finasteride, you may bring up to one month’s supply, judged by the dosage directions, without applying for anything in advance. Customs confirms the items at the border. MHLW’s Q&A gives a simple example: for a medicine taken as one tablet a day, one month means up to 30 tablets.

The calculation is based on the directions on the package or insert. According to MHLW, if the dosage cannot be confirmed from the labelling, an Import Confirmation is required even for a small quantity. Keep your tablets in their original, labelled packaging.

More than one month: the Import Confirmation certificate

To bring more than one month’s supply, you must apply for an Import Confirmation (Yunyu Kakunin-sho). You need to receive it before you leave home and show it to customs on arrival. This certificate replaced the older Yakkan Shomei from 1 September 2020. Since 1 July 2025, online application through the MHLW Import Confirmation Information System has been the standard route for personal-use imports.

For self-use applications, you need a recently issued prescription or doctor’s letter. MHLW asks that it show your name, the product name, the clinic or hospital name and the doctor’s name. MHLW’s Q&A states that a complete online application is usually processed in about two to three business days. Each certificate covers a single import, so repeat shipments need new applications.

Which office handles your application depends on your arrival point. The Kanto-Shin’etsu Regional Bureau of Health and Welfare covers arrivals such as Narita and Haneda. The Kinki Regional Bureau covers Kansai, Chubu Centrair, Fukuoka, Naha and other areas.

Common Misconceptions About Carrying Finasteride Into Japan

“A long prescription from my home doctor lets me bring a longer supply.” It does not. The one-month allowance applies no matter how many months your prescription covers. Your prescription supports an Import Confirmation application but does not replace one.

“Splitting a large order into small parcels avoids the limit.” According to MHLW’s Q&A, splitting a shipment does not avoid the rule. If one importer’s total of the same product within the period (one month for prescription drugs) exceeds the limit, an Import Confirmation is still required.

“Anything sold online as finasteride is the same medicine.” MHLW warns that personally imported medicines have not been checked for quality under Japanese law and may be counterfeit or made in unhygienic conditions. They are also not covered by Japan’s relief system for adverse drug reactions if serious harm occurs.

For longer stays, a local prescription means you do not need repeated Import Confirmation applications. It also gives you a doctor in Japan who can review how you are responding.

Cost in Tokyo

Finasteride for hair loss is self-pay in Japan. At the time of writing, the clinic group’s finasteride page lists finasteride (imported product), 30 tablets, at 2,490 yen. The group’s price list shows a standard initial or follow-up consultation fee of 3,300 yen (tax included).

Before, during and after your consultation

Bring your current packaging, or note the product name and strength, along with any prescription records from home. The doctor will confirm the diagnosis, review your history and discuss dosing within the Japanese label range of 0.2 mg to 1 mg daily.

Finasteride is a daily tablet, so there is no downtime. The Japanese guideline advises continuing for at least about six months before judging the effect. Follow-up visits cover response, side effects and repeat prescriptions.

Who is a good candidate, and who should avoid it

  • Suitable: adult men with male pattern hair loss who want to slow its progression. Japanese labelling notes that safety and efficacy under age 20 have not been established.
  • Not suitable: women, particularly women who are or may become pregnant. The US label advises that pregnant women should not handle crushed or broken tablets, because absorbed finasteride may affect the development of a male fetus.
  • Discuss first: men with liver problems, and men who have regular prostate checks.

In the US label’s 12-month trials, drug-related decreased libido was reported in 1.8% of men on finasteride versus 1.3% on placebo. Erectile dysfunction was reported in 1.3% versus 0.7%. Post-marketing reports also include depression and suicidal thoughts, so report any mood changes to your doctor promptly.

Q. How many finasteride tablets can I bring into Japan without paperwork?

Q. Can my family post finasteride to me in Japan?
A parcel sent to you counts as a personal import, so the same one-month limit applies. Have it addressed to you at your home, because MHLW requires an Import Confirmation, regardless of quantity, for parcels sent to a company address. Several parcels within one month count towards the same total.
Q. Can I bring topical minoxidil as well?
According to MHLW’s Q&A, minoxidil products stronger than 5% are limited to one month’s supply. Products at 5% or below are limited to two months’ supply. Larger quantities need an Import Confirmation.
Q. Can I donate blood in Japan while taking finasteride?
Not while you are taking it. The Japanese Red Cross Society’s medication guidance lists finasteride products as not eligible until one month after stopping, and dutasteride products until six months after stopping.
Q. Will finasteride affect my health-check results?
Finasteride lowers PSA, a blood marker used in prostate cancer screening. The Japanese guideline reports a fall of about 50% and suggests doubling the measured value as a guide. Tell the doctor at any health check that you take finasteride.
