A fat dissolving injection is a series of small injections placed directly into a subcutaneous fat pad, using a solution intended to break down fat cells locally. The most extensively studied active ingredient is deoxycholic acid, a bile acid the human body produces naturally to help absorb dietary fat.

How deoxycholic acid works

According to the US prescribing information for the deoxycholic acid product KYBELLA, the drug is cytolytic — when injected into tissue, it physically disrupts the fat cell membrane. The destroyed cells are then cleared by the body’s normal inflammatory and metabolic processes over the following weeks.

Because the cells are destroyed rather than shrunk, the reduction is intended to be lasting, provided overall body weight remains stable. This is a different mechanism from skin tightening devices, which act on collagen and connective tissue rather than on fat volume.

Approval status in Japan and abroad — please read this carefully

Deoxycholic acid injection received its initial US Food and Drug Administration approval in 2015, indicated for the improvement of moderate to severe fullness associated with submental fat (under the chin) in adults. The same label states plainly that safe and effective use outside the submental region has not been established and is not recommended.

US approval is not Japanese approval. In Japan, fat dissolving injection products are provided as self-pay (自由診療) treatment using preparations that have not been approved under Japan’s Pharmaceuticals and Medical Devices Act, typically obtained through a physician’s personal import.

Japan’s Ministry of Health, Labour and Welfare Medical Advertising Guideline (last revised 13 September 2024) requires clinics using unapproved drugs in self-pay care to disclose, among other things, that the product is unapproved, how it was obtained, its overseas approval status, and — importantly — that any resulting health damage falls outside the national Relief System for Sufferers from Adverse Drug Reactions. A clinic that does not tell you this is not meeting the guideline.

In Japanese aesthetic practice, fat dissolving injections are most often requested for:

  • Under the chin (submental area) — the “double chin”; the only area with regulator-reviewed phase 3 evidence
  • Lower face, jawline and cheeks — for softening a heavy or rounded facial contour
  • Upper arms
  • Abdomen and flanks
  • Inner and outer thighs, and above the knees
  • Bra line and upper back

Be aware of the evidence gradient here. Treatment of body areas other than under the chin is off-label use, supported by clinical experience rather than by the large randomised trials that underpin the submental indication. A responsible consultation should say so rather than presenting every area as equally proven.

In the two pooled double-blind, placebo-controlled trials submitted to the FDA (513 treated subjects versus 506 placebo), 70.0 percent of treated patients in one trial achieved at least a one-grade improvement on the combined clinician and patient rating scale, compared with roughly 18 to 22 percent on placebo.

In an MRI sub-study of 449 subjects, 43 percent of treated patients showed at least a 10 percent reduction in submental fat volume, versus 5 percent of placebo patients. Note what that figure means: measurable, meaningful reduction — not the elimination of a fat pad.

Results build slowly. Fifty-nine percent of trial subjects required all six permitted treatment sessions, spaced at least one month apart. Expecting a visible change after a single session is the single most common source of disappointment.

On durability, a three-year follow-up of REFINE phase 3 participants (224 patients) published in Aesthetic Surgery Journal in 2021 reported that 82.4 percent of treated responders maintained their improvement at year three, and clinical reports indicate patient satisfaction was largely sustained over the same period.

Fat dissolving injection is self-pay treatment in Japan and is not covered by national health insurance, so the full cost is borne by the patient. Pricing in Tokyo is usually structured per vial, per treatment area, or as a multi-session course.

Three factors drive the total more than the headline per-session figure: the size of the area (an abdomen requires far more product than a submental pocket), the number of sessions you actually need, and the product used, since concentration and formulation vary considerably between preparations.

At consultation, the treating physician should assess whether your concern is genuinely subcutaneous fat rather than skin laxity, muscle bulk, or salivary gland prominence — these look similar to the untrained eye but respond to entirely different treatments.

The injection itself takes roughly 15 to 20 minutes. Topical or local anaesthesia and cold packs were permitted during the clinical trials and are commonly used.

Downtime is real and should not be minimised. In the pooled FDA trial data, injection site swelling occurred in 87 percent of treated subjects, bruising in 72 percent, pain in 70 percent and numbness in 66 percent. Reactions persisting beyond 30 days included numbness in 42 percent and swelling in 20 percent.

Less common but more serious risks in the same dataset included marginal mandibular nerve paresis — a temporarily uneven smile — in 4 percent, and difficulty swallowing in 2 percent. Injections placed too superficially can cause skin ulceration and necrosis. This is precisely why anatomical precision, and a physician who knows where the nerve and salivary glands sit, matters more than the product brand.

Who is a good candidate — and who should avoid it

Good candidates typically have a discrete, pinchable fat pocket, reasonably good skin elasticity, a stable body weight, and realistic expectations about gradual change over several months.

The treatment is contraindicated where there is infection at the injection site. It should be approached with particular caution in patients on antiplatelet or anticoagulant therapy or with clotting abnormalities, and in those with pre-existing swallowing difficulty. It is not a weight-loss treatment, and it does not correct sagging skin.

What People Often Get Wrong

Misconception 1: “It’s FDA-approved, so it must be approved in Japan and safe for any body area.” Two errors in one sentence. FDA approval covers submental fat in adults only, and the label explicitly states that use outside the submental region has not been established. Separately, FDA approval carries no weight under Japanese law — products used here are unapproved under the PMD Act, and Japan’s Medical Advertising Guideline requires clinics to state that adverse events are outside the national relief system.

Misconception 2: “It’s a lunchtime treatment with no downtime.” The manufacturer’s own trial data contradicts this: swelling in 87 percent, bruising in 72 percent, and numbness lasting more than 30 days in 42 percent of subjects. Plan around a visible recovery period, particularly for facial treatment.

These distinctions matter commercially as well as clinically. Consultations about cosmetic medical services logged by Japan’s National Consumer Affairs Center rose to 10,744 in FY2024 from 6,281 the previous year, with inadequate explanation of risk a recurring theme.
