Following a 2022 revision, online initial consultations were made a permanent option for a range of conditions, including cosmetic and lifestyle-related concerns such as AGA, rather than the temporary, pandemic-era exception that existed before.

In practice, this means a dermatologist or plastic surgeon can review your scalp condition through a live video consultation, take a medical history, and issue a prescription without an in-person visit for many AGA cases. However, MHLW guidance requires the consultation to be conducted by real-time video rather than a text chat or a one-off phone call; the temporary telephone-only allowances used during the COVID-19 period have since been withdrawn, so a platform offering only asynchronous questionnaires is not operating within current guidance.

What a Doctor Can and Cannot Do Remotely

A remote consultation can cover history-taking, visual assessment of hair thinning patterns, and prescribing of approved oral or topical medication.
