Morpheus8 is a fractional radiofrequency microneedling device made by InMode. Very fine, insulated needles pass through the skin surface and deliver radiofrequency energy into the deeper layers, creating controlled zones of thermal injury that prompt the skin to remodel over the following weeks.

The distinguishing feature is depth control. Because the energy is released from the needle tips rather than applied to the surface, the practitioner can select how deep the effect is placed — on the clinic’s own treatment page, energy delivery of up to approximately 7.5 mm is described, depending on the applicator and the area being treated.

This matters clinically. A narrative review published in Plastic and Reconstructive Surgery – Global Open in March 2026 notes that RF microneedling produces “an electrothermal effect as opposed to a photothermal effect,” meaning that melanin is not the target chromophore as it is with many lasers. The review’s authors concluded that RF microneedling is safe for all skin types on that basis.

Regulatory status: the United States and Japan are not the same

In the United States, the FDA’s 510(k) database records clearance K192695 for the InMode System with the Morpheus8 (Fractora) Applicators, with a decision date of 27 December 2019, and a further clearance, K240017, for the Morpheus8 90 Applicator with a decision date of 13 June 2024. InMode announced the latter on 17 July 2024 as the first fractional RF microneedling clearance for contraction of soft tissue.

In Japan, Morpheus8 has not received approval under the Pharmaceutical and Medical Device Act. It is brought in through the physician-import route, which the Kinki Regional Bureau of Health and Welfare describes as importing for use by the physician “under their own responsibility” for their own patients, and only where no equivalent approved domestic product is already available.

Japan’s Ministry of Health, Labour and Welfare sets out how self-pay treatments using unapproved devices must be presented to the public in its Medical Advertising Guidelines, supported by a case-explanation document most recently issued on 30 March 2026. In practice, a clinic offering Morpheus8 in Japan should be telling you plainly that the device is unapproved domestically, how it was obtained, and what is known about its safety overseas.

The most frequently cited outcome data concerns acne.

A retrospective review published in Aesthetic Surgery Journal in 2025 compared three approaches across 356 patients: isotretinoin (n=128), ablative laser over six sessions (n=89), and fractional RF microneedling with Morpheus8 over three sessions (n=139).

Among the subset who completed three-year follow-up, relapse rates were 36 of 54 (67%) in the isotretinoin group, 12 of 16 (75%) in the laser group, and 7 of 29 (24%) in the fractional RF microneedling group. These are encouraging figures, but they come from a retrospective design with small follow-up subsets, so they should be read as suggestive rather than definitive.

For skin of colour specifically, the 2026 PRS Global Open review cites a study in which 58% of patients with Fitzpatrick types III–V showed moderate improvement in acne scarring, 9% good improvement, and 3% very good improvement. The review also recommends beginning at conservative energy settings in Fitzpatrick IV–VI and increasing gradually across a treatment course.

Realistically, clinical reports indicate that collagen remodelling continues for roughly three months after treatment, so results build progressively rather than appearing immediately. No device can guarantee a specific degree of tightening in any individual.

Documented risks include mild swelling, tenderness, bruising, pinpoint scabs and temporary roughness, along with pigmentary change. Less common but more serious complications include marked swelling, blistering and infection. Who is a good candidate, and who is not

Morpheus8 tends to suit people with mild to moderate laxity, textural irregularity or atrophic acne scarring who want remodelling without surgery and can commit to a course rather than a single visit. Because the mechanism is electrothermal rather than light-based, it is also considered across a broad range of skin tones, with more conservative settings recommended for Fitzpatrick IV–VI.

It is less appropriate where laxity is pronounced enough that only surgery will meaningfully address it, and it is not the right choice during active skin infection, over recently treated or inflamed skin, or in pregnancy. Patients with implanted electronic devices, a history of keloid scarring, or a melasma history should raise this at consultation, as thermal treatments can provoke pigmentary responses in susceptible skin.

What People Often Get Wrong About Morpheus8

“It is FDA-approved, so it must be approved in Japan too.”

These are unrelated regulatory systems. The FDA record shows 510(k) clearances — a substantial-equivalence pathway, not a premarket approval — and neither of those clearances has any bearing on Japanese status. In Japan the device remains unapproved and is used under the physician-import route, which is precisely why a compliant clinic must disclose that fact to you.

“Stacking devices in one session must work better.”

Not necessarily. A 2025 meta-analysis in Cureus pooled five randomised split-face trials covering 116 patients (232 facial sides) and found that adding fractional microneedling radiofrequency to non-ablative fractional laser produced a pooled odds ratio of 1.22 (95% CI 0.70–2.12, p=0.48) for achieving 50–75% improvement in atrophic acne scars — no statistically significant advantage over the laser alone, and no significant difference in side effects. Well-chosen settings on one appropriate device generally matter more than the number of devices used.
